How secure are smaller sized county courthouses?

Some counties actually have their jail on one floor of or attached to their building. Is this a safe and secure way to house inmates? How are they escorted to the courtrooms in the same building? Are restraints (handcuffs/leg irons) used, or are the inmates simply escorted unrestrained by officers to to the courtroom?
